Final report of the four-year effect study of Communities that Care in the Netherlands funded by ZonMw (project 120610017), the Dutch-language source behind Steketee et al. (2013) and Jonkman et al. (2015). Method: five experimental and five matched control neighbourhoods in Capelle aan den IJssel, Gouda, Middelburg, Spijkenisse and Zwijndrecht, followed from January 2008 to December 2011 with school surveys before and after, a youth panel, milestones-and-benchmarks ratings of programme integrity, key-informant questionnaires and programme registration; planned as a randomised trial, it became quasi-experimental because not all cities accepted random assignment. Results: phases one to four were implemented as intended everywhere; cooperation became more efficient and more evidence-oriented in the CTC neighbourhoods, whose key informants invested more time in prevention and reported healthier adult norms on alcohol and cannabis; more programmes ran in CTC areas, yet control areas caught up and in two cities received the same offer, and programme reach was barely registered. Neither the pre-post comparison nor the longitudinal panel showed any effect on risk or protective factors or on substance use, delinquency, sexual problem behaviour or depression. Conclusion: CtC works as a strategic steering instrument, but the design could only have detected large or medium effects.
