Overview article in German in the Bundesgesundheitsblatt (2023, vol. 66, no. 7, pp. 774–783; open access) presenting Communities That Care as an answer to three persistent challenges of community health promotion in Germany: cross-sector collaboration, evidence-based practice and sustainable implementation. CTC is described as a systemic multi-level strategy aimed at preventing alcohol and drug misuse, violence, delinquency, school drop-out and depressive symptoms in adolescents. Developed in the United States, where it proved effective and cost-effective, it has been transferred to Germany, where a replication study is testing whether the cost-effectiveness holds. The paper walks through the five-phase process model, the cross-sector coalition whose members receive several years of coaching and training, and the validated instruments that support the process, the CTC youth survey and the evidence register Grüne Liste Prävention. Evidence-based measures are chosen from local data and needs and implemented with attention to context, so as to reduce risk factors, strengthen protective factors and improve young people’s health. As a systemic intervention CTC integrates existing local structures through new decision-making and development bodies, pooling resources and creating transparency.
