EMCDDA Papers publication reviewing the evidence on Communities That Care as a drug-prevention approach, written by Laura Amato, Zuzana Mitrova and Marina Davoli (Cochrane Drugs and Alcohol Group). Following Cochrane methods, the authors searched the Cochrane Drugs and Alcohol Group register, CENTRAL, MEDLINE and EMBASE to September 2015 for randomised, controlled prospective and quasi-experimental evaluations of CTC coalitions with young people aged 12 to 25, screening 1,181 records and including 18 reports. These comprise the US Community Youth Development Study (one cluster RCT reported in twelve papers), an underpowered Australian community-action RCT, the English before-and-after evaluation by Crow and colleagues, two quasi-experimental Pennsylvania studies by Feinberg and colleagues, and an EMCDDA report on European implementation. Outcomes were too heterogeneous for meta-analysis, so results are described narratively: the US trial showed lower risk and lower initiation of alcohol, tobacco, delinquency and violence through grades 10 and 12, but no effect on current prevalence in grade 12; the Australian trial gave little evidence of effect. The paper concludes there is some evidence of effectiveness outside the EU, that cultural adaptation matters, and that a European multi-site randomised trial would be needed.
