Congress contribution to the 74th Scientific Congress of the German Public Health Service (Erlangen, April 2025), published as an abstract in Das Gesundheitswesen. Using the FINDER programmes Schools That Care and Weitblick as the example, the contribution shows how the Communities That Care logic is carried into a single school and how the anonymous pupil survey on risk and protective factors can be linked with municipal data, so that school and municipality plan their prevention on the same evidence base. It is a practice and concept paper without outcome data of its own; the external evaluation of Weitblick by Hannover Medical School is published separately (study protocol Röding et al., 2025; school-level analyses Röding et al., 2026).
